Profiles of Chinese Heritage Schools/Programs in the United States

This survey will help us build searchable directory and program profiles of community-based Chinese heritage schools/Programs in the United States. Your participation will better inform the general public about the demographics, curriculum, strengths and needs of your school.
Important tips:
  • This survey will take approximately 30 minutes to complete after you have the information gathered.
  • Please provide the most complete information with as many details as possible to give readers the fullest understanding of your institution.
  • For your convenience, this online survey has three language versions: English, simplified Chinese and traditional Chinese. Please note that the information to be shown in the Directory and Profiles of Chinese schools on the CHELER website will be the language you chose to complete the questionnaire.
  • You can navigate between the individual parts of the survey using the menu options in the left column. Please note that, once you click on Contact Information or Contact Person, you must complete those sections before you move to other sections in the form.
  • There are two parts to the online survey. We hope you will be able to complete it in one sitting. If necessary, you can complete Part I or a section of the survey and return at a later time to complete the rest.
  • Please be sure to save constantly as you fill out the survey. At the bottom of each page, you will see a “Save & Next” button. Click this to save.
  • All the information marked with a red star * needs to be completed before you can move on to the next page.
  • Please make note of the email address and the password you created when beginning the process and use this same email address and password to access your submitted profile information at a later date using the “Edit” function of our database.
  • You will receive an e-mail confirmation soon after you click on “Finish” at the end of the survey. Please check your spam mail box in case you do not receive your confirmation e-mail.  Also, please add our address [email protected] in your contact list for any future information from us.
